Voyage Healthcare’s Anticoagulation Clinic offers dedicated monitoring services for patients taking Warfarin (Coumadin), a medication commonly referred to as a “blood thinner.” Voyage Healthcare’s caring team of experienced nurses will assess each patient’s anticoagulation status and associated risk factors to develop a personalized treatment plan. We will work closely with the patient and our healthcare team to keep our patients safe and ensure the medication is working as it should.

Routine blood tests are an essential component of warfarin use. These quick tests allow our healthcare team to measure the blood’s clotting ability by monitoring the INR level (International Normalized Ratio) on a regular basis. Patients should expect to be seen in our Anticoagulation Clinic at least once a month. More frequent visits may be necessary depending on changes to the patients’ health status, medications, or lifestyle.
