Our Maple Grove, Osseo and Crystal offices will close at 5:00 pm Monday – Friday.
Our Maple Grove office will continue to be open Saturday 8:30 am - 12:00 noon for Family Medicine appointments.

Voyage Healthcare now offers an Acute Illness Clinic within our Osseo office and will no longer be seeing patients in the tents in the Osseo parking lot. This transition is occurring as a way to protect our Providers and Staff from the summer weather/heat, as well as a way to provide our patients with a more thorough exam. If you're unable to complete this form ahead of time, we ask that you arrive a few minutes ahead of your scheduled appointment time to complete the form in the office.
